About Littmann Classic 3

Who is the Classic III for?

It is the stethoscope for people who examine a bit of everything. The large side of the chestpiece covers adults, the small side works on children and thin patients, and both carry a tunable diaphragm: light contact emphasises low-frequency sounds such as a third heart sound or a mitral rumble, firm contact brings out higher-pitched murmurs and breath sounds. The small side also converts to a traditional open bell by swapping the diaphragm for the supplied non-chill sleeve. That range, combined with a low weight you can wear through a long shift and single-lumen tubing that keeps rubbing noise down, is why it is the default instrument in family medicine, in nursing, in emergency work and on every medical school reading list. It is sensitive enough to describe a murmur properly; only routine cardiology work justifies stepping up to a Cardiology IV.

How does it differ from the Classic II and the Cardiology IV?

Against the older Classic II family, the important change is that both sides of the Classic III are tunable, not just one, so you can vary frequency response wherever you place the chestpiece instead of flipping the head. Its tubing also resists skin oils and alcohol better, which is what usually stiffens and cracks an older stethoscope after a few years of daily disinfection, and the whole chestpiece is a single machined piece that is easy to wipe clean. Against the Cardiology IV, the layout is identical but the chestpiece is less sensitive and the tubing lighter: the difference is audible on faint low-frequency murmurs and on obese or muscular patients, and inaudible on most routine consultations. The Cardiology IV is also heavier and costs roughly twice as much, from 1,470 RON.

Which colour and finish should you choose?

Every Classic III in this category sounds the same; the 640 to 750 RON spread is finish, not performance. The plain-tube versions are the entry point: Caribbean Blue 5623 at 640 RON, Azure Blue 5630 at 645 RON, raspberry 5648 at 675 RON. Special chestpiece finishes sit at 705 to 750 RON — the rainbow finishes 5806 and 5807, the smoke finishes 5872 and 5873, the black-chestpiece burgundy 5868, the copper chocolate 5809 and the satin stainless 5910C. Practical guidance: smoke, black and satin finishes hide fingerprints and fine scratches and reflect little theatre light, mirror and copper finishes stay eye-catching but show handling wear, and a distinctive tubing colour is the simplest way to identify your own instrument in a team where everyone carries the same model. The spare-parts kit you will eventually need is the same for all of them.

Frequently asked questions

Yes, that is the point of the dual-sided chestpiece: the large side for adults, the small side for children and thin patients. The exception is newborns, where the small side still bridges the ribs and lets air leak under the rim. For neonatology and premature babies you need the much smaller chestpiece of the Classic II Infant.

Place the chestpiece flat and press only lightly to hear low-frequency sounds — the membrane is then free to move. Press firmly, so the diaphragm is held against its ring, to bring out higher frequencies. Practise switching between the two on the same spot: hearing a murmur change character as you change pressure is what tells you which frequency band it occupies.

Only if you prefer a classic bell for very low-frequency sounds or work on bony chests where a diaphragm will not seal. Unclip the paediatric diaphragm and press the supplied non-chill sleeve into the rim; the change takes seconds and is reversible. Most users leave the tunable diaphragm in place and vary pressure instead, which achieves much the same result.

The standard length suits most clinicians: it puts the chestpiece comfortably on a supine patient's chest while the headset sits naturally. Shorter tubing loses a little less sound and is tidier to wear, but forces you to lean closer; longer tubing gives you reach on a bed or a trolley. Any length difference matters far less than a good eartip seal.
