When do you need an infant stethoscope rather than a paediatric one?

Auscultation works only if the chestpiece seals against skin. On a newborn or a premature baby, an adult or even a paediatric chestpiece bridges across the ribs, so air leaks in under the rim and you hear the room instead of the heart. The Classic II Infant has the smallest chestpiece Littmann makes for exactly that reason: it fits between the ribs of a neonate, sits inside an incubator port and lets you separate heart sounds from breath sounds on a chest that measures a few centimetres. Use it for delivery-room checks, newborn examinations, neonatal intensive care and the first weeks of paediatric follow-up. Once the child grows into infancy and toddler years, the Classic II Pediatric becomes the better size, and school-age children can be examined with an adult Classic III.

Which Classic II Infant model should you buy?

All versions here are the same instrument; the price follows the finish and the tubing colour. The black 2114 is the least expensive at 609 RON, the Caribbean Blue 2124 is 610 RON and the red 2114R is 615 RON. The raspberry version with the rainbow chestpiece finish, 2157, costs 645 RON and is popular in paediatric practice for a simple reason: a colourful chestpiece makes the instrument less threatening for a toddler or an older sibling watching the examination. Beyond looks, the choice is about identification. A distinctive colour is the easiest way to keep track of your own stethoscope in a neonatal unit where several identical black ones hang on the same trolley. Acoustically there is nothing to compare between them, so buy the colour you will recognise instantly.

How do you get a clean recording on a newborn?

Warm the chestpiece in your hand first — a cold rim makes a baby cry, and a crying baby is impossible to auscultate. Work in the quietest moment you can find, ideally while the baby is asleep or feeding, and place the chestpiece with the whole rim in contact, using no more pressure than it takes to close the gap. Listen through clothing only if you cannot avoid it: fabric rubbing under the rim adds more noise than any stethoscope can filter out. Between patients, wipe the chestpiece and tubing with a 70% alcohol wipe, keep hand gel off the tubing so it does not stiffen, and never immerse the instrument. When the eartips harden or the diaphragm rim is damaged, the 40013 kit at 90 RON replaces the parts that wear first.
